This form should take 10 minutes to complete and it will take about 2 working days to get you registered. If at any point you find this form too hard, you can visit the practice to register a different way.
If you are homeless or have no fixed abode, please contact the practice.
Finally, if there is a medical emergency, call 999 and if it's less urgent but still important, go to NHS 111 online, or call 111.
Register as a New Patient
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so it would help if you could provide proof by way of a recent utility bill.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both forms are available from reception or online via the links below.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
Practice Boundary
Every patient has a GP that they are assigned to but Patients have a right to see a specific practitioner if they so wish and they should tell the receptionist of their preference when making an appointment. However, while the practice will endeavour to comply with their wishes, there will be occasions when it will not be possible.
The practice has a policy that we do not discriminate in terms of race, creed, age, disability etc.
Identification in order to register you. For adults, this means a form of photo I.D (passport or driving license) and proof of address (utility bill or bank statement). If a child or young person under the age of 18 is registering, we only require a birth certificate. Please note that you can still register without ID or address.
Temporary Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
